We are glad you are here. Before your student can enroll in our school system, you must complete/provide the following:

OLR (Online Registration from Infinite Campus Parent Portal). Clickhereto access.

• Records Request (Parent approval to request school records from previous school/s). Click here to access.

• 2 Proofs of Residency (review attachment for specific approved documents)

• Birth Certificate/Proof of Custody/Court papers related to guardianship

• Social Security Card

• Updated/Current physical & KY immunizations

• Current grades, schedule, and transcript

 - must have been completed within one year (12 months) prior to enrollment date
 - EXCLUDE FROM SCHOOL UNTIL TURNED IN

 Immunization 

 - up-to-date on a KY Immunization Certificate
 - must be verified by a school nurse
 - EXCLUDE FROM SCHOOL UNTIL VERIFIED

Vision
 - out-of-state transfers ONLY
 - must be on a KY form
 - documentatation provided within 30 days of enrollment
